ACCP Venous Thromboembolism (VTE) Prophylaxis Performance Improvement Project
The ACCP and its collaborators have engaged in developing a performance improvement tool that addresses the prevention of deep vein thrombosis (DVT) and pulmonary emboli (PE) in moderate-to-high risk patient groups in the medical intensive care unit (MICU), under the care of the MICU team or in other medical monitored situations. This tool will provide participants with the ability to address barriers to the clinical implementation of NQF-endorsed performance measures.

PAVE PI CME (Partnership for Adult Vaccination and Education)
The overall objective of this PI CME activity is to improve vaccination screening and administration practices and rates in adults 19 years and older with regard to the following vaccines: Hepatitis A and B, HPV, Influenza, Meningococcal, PPSV, Td/Tdap, Varicella, and Zoster. Specifically, this includes the identification of all adult patients whom the ACIP recommends should currently receive an adult vaccination; offering those adults the appropriate vaccinations; appropriately counseling those adults regarding the risks and benefits of their recommended vaccinations; and finally, ensuring that those who are eligible are indeed vaccinated.

SHM: Glycemic Control Mentored Implementation Program
SHM’s Glycemic Control Mentored Implementation (GCMI) Program aims to support sites in optimizing the care of inpatients with hyperglycemia and diabetes, and in preventing hypoglycemia. Each of the selected sites will receive a year of mentoring from national experts to help optimize glycemic control in their hospital. Participation in this program provides access to a comprehensive data registry that allows for performance tracking and benchmarking against comparable institutions. The range of topics addressed through this program includes: subcutaneous insulin protocols, transition from subcutaneous to infusion, infusion insulin, care coordination, improving follow up care, and having a good hypoglycemia management and prevention protocol.

SHM: Project BOOST (Better Outcomes for Older Adults through Safe Transitions)
SHM’s BOOST Mentored Implementation Program provides yearlong mentoring and coaching from national experts to redesign discharge processes and improve patient safety. Participation in this program provides access to a comprehensive data registry that allows for performance tracking and benchmarking against comparable institutions. By improving hospital discharge processes, Project BOOST aims to reduce 30 day readmission rates for general medicine patients, improve patient satisfaction and H-CAHPS scores related to discharge, improve flow of information between hospital and outpatient providers and improve patient and family preparation for discharge.

SHM: Venous Thromboembolism Prevention Collaborative (VTE PC) Mentored Implementation Program
SHM’s VTE-PC Mentored Implementation Program pairs mentors with VTE and quality improvement expertise with hospitalists who are leading local improvement projects. Participants receive practical assistance on topics central to designing, evaluating, implementing and sustaining a VTE prevention program. Instruction and support are offered through scheduled telephone calls that occur over a 15-18 month period. The program emphasizes integrating risk assessment into the work flow, layering QI interventions to maximize the proportion of patients receiving prophylaxis, and developing auditing strategies that promote rapid identification and mitigation of outliers. Participation in this program provides access to a data registry that allows for performance tracking and benchmarking against comparable institutions.

UPenn COPD Performance Improvement Module
This CME-certified PI/QI module is designed to improve performance of primary care providers in effectively screening, diagnosing, and managing patients with COPD, with the goal of slowing the progressive loss of lung function and improving the patient's overall quality of life. University of Missouri Center for Health Policy: Health Literacy
This program will assist clinicians to integrate health literacy principles and techniques to improve communication with patients and increase patient understanding. The program consists of a one-day workshop which couples on-site simulation training with one-on-one planning with a health literacy expert. Survey tools and a data collection plan are provided to assist in tracking improvements over a one year period. Ongoing technical assistance is provided by program staff to physicians throughout the duration of their participation.

University of Nebraska Medical Center: NOW Coalition for Bipolar Disorder - Performance Improvement CME
The NOW Performance Improvement CME activity enables practicing clinicians to assess their performance, knowledge, and practice infrastructure in their services for bipolar disorder and offers guidance as well as tools to bridge identified gaps. Using a HIPAA-compliant, confidential platform, clinicians can evaluate how well their practice diagnoses and manages bipolar disorder, and then be guided toward resources that help to overcome barriers and initiate systems-based solutions.

PROTECT (Supporting Appropriate Immunizations Across the Age Spectrum) is a collaborative educational curriculum with the mission of improving clinician performance and patient health associated with immunizations across the age spectrum.
Nine partners, working in cooperation with a myriad of organizations, are working to address specific unmet needs and clinical gaps in the area of vaccine-preventable disease (VPD) by improving the performance of clinicians who are currently responsible for administering immunizations to applicable childhood, adolescent, and adult patient populations.

University of Oklahoma: Implementation of Chronic Kidney Disease Guidelines in Primary Care
Primary care clinicians in Oklahoma, Wisconsin, Minnesota, and Southern California will receive assistance with implementation of the National Kidney Foundation KDOQI Guidelines in the form of performance feedback, academic detailing, practical tools, quality improvement facilitation, and monthly meetings with other clinicians in the project. This assistance will be provided over a 6 month period of time by staff from regional practice-based research networks in each of the four participating states. Satisfactory completion will involve review of the performance reports and toolkit materials, attendance at one academic detailing session, collaboration with the practice facilitator to make positive changes in care processes, and attendance at monthly meetings with other participating clinicians over a period of 6 months.

This performance improvement activity is designed for physicians who treat and manage patients with post-surgical skin/soft tissue infections, catheter-related bloodstream infections, and/or ventilator associated pneumonia in a hospital setting, and want to be engaged in an activity to identify and reduce CA-MRSA occurrences.
